Chemical Name: Protriptyline HCIBrand Name: Vivactil Manufacturer: Merck and CompanyDescription: Anti-depressant of unknown mechanism. Indication and Use: Vivactil is indicated for the treatment of depression in patients who are under close medical supervision. Its activating properties make it particularly suitable for withdrawn and anergic patients. Tablets are available in 5 mg and 10 mg.

Contraindications, Warnings, and Precautions: Vivactil should not be given concomitantly with a monamine oxidase inhibiting compound. Vivactil may block the antihypertensive effect of guanethidine or similarly acting compounds. It should also be used with caution in patients with a history of seizures, and because of its autonomic activity, in patients with tendency to urinary retention, or increased intraocular tension. Vivactil should be used with caution in elderly patients and patients with cardiovascular problems. Do not use with alcohol. Do not use during pregnancy.

Adverse Reactions: Vivactil is more likely to aggravate agitation and anxiety and produce cardiovascular reactions such as increased heart rate and hypotension. Possible side effects may include palpitation, insomnia, panic, nightmares, headache, weakness, fatigue, dry mouth, edema, abdominal cramps, and black tongue. Abrupt withdrawal of long term therapy may cause nausea, headache, and malaise (this does not indicate addiction).

Dosing Range: Dosing should be initiated at a low level and increased gradually according to response and tolerance. Adult dosage is 15-40 mg a day divided into 3 or 4 doses. Doses above 60 mg/day are not recommended.

Vivactil (Protriptyline HCI): This antidepressant is under the tricyclic class. Vivactil is used to treat depression as well as nerve pain, insomnia, bed-wetting in children, and may take 2-3 weeks before the full effects of this medication are noticed. Vivactil should be taken at the same time each day. Do not stop taking this medication without your doctor's approval. Nausea, headache or fatigue can occur if the drug is suddenly stopped.

Common side effects: drowsiness, dizziness, increased sun sensitivity, or blurred vision. heartburn, loss of appetite, dry mouth, strange taste in mouth, anxiety, restlessness or sweating. These medications should subside as your body adjusts to the medication.

Rare side effects: chest pain, rapid heart rate, difficulty urinating, nightmares, ringing in the ears, excessive sedation or uncoordinated movements. You should notify your doctor if you obtain these side effects while taking this medication. Symptoms of an allergic reaction include rash, itching, swelling, dizziness, or trouble breathing, If you obtain these side effects, you should seek immediate medical attention.

Precautions: Before taking this medication, you should tell you physician your medical history, especially glaucoma, prostate problems, decreased urine output, thyroid disease, breathing problems, seizure problems, alcohol use, heart disease, mental/emotional problems, liver or kidney disease and any drug allergies.

Warnings: Avoid alcohol while taking this medication. Do not drive or operate heavy machinery until you know how your body will react to this medication. Limit exposure to hot weather as this medication may contribute to heat stroke. Before taking this medication, inform your doctor if you are pregnant, planning on becoming pregnant, or breast feeding. You should inform your doctor before combining this medication with any other medication, including over-the-counter medication. If you miss a dose, and it is almost time for your next dose, skip the missed dose and continue with your schedule. Do not double up on this medication. This medication is available only by prescription.
